Our Approach
Our approach to “Keeping loved ones at home” is based on two themes: client-centricity and client-caregiver alignment.
Client-centricity
Empathize: We start by putting them at the center of our understanding and listen critically to and through their words. This establishes our contextual framework for the next step. We then augment the context with other information shared by family members and professionals connected to them as relevant.
Customize: Once the client’s needs framework is baselined and captured, we then design the baseline of care and related services necessary and tailor it.
Serve: We then operationalize the system of professionals, practices and partners accordingly.
Client-caregiver alignment is a continuous process and a “team sport.” As our clients’ care requirements evolve, we make sure that we can deliver care today while preparing for inevitable change. That means creating a deep and reliable “bench” of professionals to draw from.
Identify: A dynamic activity, we have defined various roles and related competencies necessary to provide a range of care services — companion, personal and complex care.
Empower: We connect our caregivers to our ecosystem of relevant information, leading practices and people.
Support: We provide opportunities for continuous education and training to help our caregivers enhance their skills to enable them to best serve our clients.
All throughout, we work on assuring a collaborative administrative system—which is more than communication—between clients, their families, caregivers and health care partners.
